I declare that I have been informed that:

1. the administrator of the personal data processed as part of the recruitment process for the language course is the University of Zielona Góra, 65-417 Zielona Góra, ul. Licealna 9; tel. 683282000, fax: 683270735,
2. the administrator has appointed a Data Protection Officer, who can be contacted regarding the protection of your personal data by e-mail: IOD@adm.uz.zgora.pl or by phone: 603474724,
3. personal data, official data and contact data will be processed for the purpose of the current recruitment procedure on the basis of the consent given (Art. 6(1)(a) RODO),
4. the data subject has the right to withdraw consent at any time without affecting the lawfulness of the processing carried out on the basis of consent before its withdrawal,
5. the recipients of the data are authorised employees of the University of Zielona Góra in charge of preparing contracts and settling financial issues,
6. the data collected in the recruitment processes will be stored for the duration of the course as well as for the purpose of processing possible claims and for archiving purposes,
7. the data subject has the right to access their personal data, to request their rectification or erasure. By submitting a request for erasure, the data subject shall be deemed to have opted out of the recruitment process conducted by the University Centre for Language Education. In addition, the data subject has the right to request the restriction of processing in the cases set out in Article 18 of the DPA;
8. the data subject has the right to lodge a complaint with the President of the Data Protection Authority against the unlawful processing of their personal data. This authority will be competent to hear the complaint except that the right to lodge a complaint concerns only the lawfulness of the processing of personal data and does not concern the course of the recruitment process;
9. providing the data contained in the recruitment documents is not obligatory, but it is a condition for applying for a language course at the University Centre for Language Education.
